Financial Policy
Payment is due in full at time of service unless you are approved for a Payment Plan or have verifiable benefits available through an insurance plan. INSURANCE Our office understands the value of insurance benefits to our patients. Once benefits have been verified, in order for us to accept assignment from your insurance company a valid credit card will be securely placed on file. We will collect any estimated copay, deductible and difference in fees at the time of service. Any remaining balance after 45 days will be automatically billed to your credit card. To insure maximum insurance benefits be sure to read your dental plan benefit booklet provided by the insurance company prior to your appointment and discuss all concerns with your insurance company. USUAL AND CUSTOMARY RATES Our practice is committed to providing the best treatment for our patients. We charge according to the costs associated with your treatment and for the expert care you deserve. Your are responsible for payment regardless of any insurance company's arbitrary determination of usual and customary rates. MISSED APPOINTMENTS Appointments are valuable blocks of time. When an appointment is broken or cancelled on short notice (less than 48 hours) it prevents us from helping someone else. Wasted appointment time also results in higher health care fees. In order to control costs for all our patients, we must charge a non-refundable $70.00 cancellation fee for all appointments that are cancelled with less than 48-business-hours notification (Monday - Friday). As a courtesy to our patients, we make every effort to contact scheduled individuals to remind them of their appointment, however, keeping your appointment is your responsibility. Please help us control costs as well as serve you better by keeping all scheduled appointments.
